Bariatric Surgery Insurance & Financing

For weight loss surgery patients:

The first step is to verify your insurance policy covers weight loss surgery. Your insurance company will post this information on their website. Make sure to review it before starting the process. Ask for a copy of bariatric policy so that you are aware of what you should complete for insurance approval.

Sometimes, your insurance company may not cover weight loss surgery. We also offer financing for those patients who do not meet the criteria and/or have an exclusion on their plan. You can learn more about finance options through Care Credit.

CPT Codes

This information may help when speaking to your insurance company. These are the CPT codes for the most common bariatric surgeries:

43775- Sleeve Gastrectomy
43770- Gastric Banding (lap band)
43644- Gastric Bypass

Insurance Requirements

  • Psychological evaluation – all insurance plans require you to have a psychological evaluation. We have two recommendations which we can provide you with.
  • 6 month diet – some medical insurance plans require you to complete a 6 month dietary and exercise plan prior to surgery. This needs to be documented, so you can see your primary care doctor for this OR you can see Dr. Kurian.
  • Nutritional assessment – all insurance plans require a nutritional assessment. You can contact us to speak with our nutritionist Meg Galligan for further details.

  • 5 year history – a couple of insurances require you to have a 5 year history of your weight.

  • Recommendation letter from your primary care provider (PCP) – most of the insurances require a letter from your PCP recommending bariatric surgery.
